(2) Complete APPENDIX I-SO to support Catholic school grades KŠ12. Complete APPENDIX
I-PKSO to support Catholic pre-school programs. Yes, you can apply for both in the same year.
Note: The Second Century Fund is a program-approved "Scholarship Organization." Although you do not need to reference our fund on your
application, please be sure to indicate the amount of your gift for scholarship organization(s).
(3) Fax your completed application(s) to the PA DCED at (717) 772-3581. Wait for an official approval letter from DCED before making an EITC contribution.
(4) Follow instructions for making an EITC contribution according to the DCED approval letter. Your total contribution amount will be stated in

To restrict your gift to your favorite school(s), please enclose a note with your contribution. Without a note from you,
your unrestricted gift will support students with the greatest financial need in the schools we support.
(5) We will promptly acknowledge receipt of your contribution according to DCED guidelines. To complete the EITC process,
you must mail or fax a copy of our official acknowledgement to the attention of the individual identified in your DCED approval letter -
and you must do so within 60 days from the date of your DCED approval letter. DCED's fax number is (717) 772-3581.
DCED will inform the PA Department of Revenue of your participation in the EITC and/or PKTC program.
Please file our original acknowledgment with your tax papers.
(6) Take satisfaction knowing that you have chosen to redirect a portion of your PA business taxes from Harrisburg to the local community by
aiding local families who choose to give their children the experience of a Catholic education.
